Waiver and Release

I, the undersigned as the parent and legal guardian of the child(ren) listed on this form, give permission for my child(ren) to participate in the youth hockey activities offered by the Denver Country Club. I fully understand and acknowledge that my child’s participation is voluntary and that there are certain substantial and inherent risks involved in sport participation. I also understand that this request does not guarantee a spot on the team.

Photo Release

I hereby grant permission to the Denver Country Club the right to use, reproduce, and/or distribute photographs, films, videotapes, and sound recordings of my child, without compensation or approval rights, for use in materials created for purposes of promoting the junior camps, programs and events at the DCC.

By signing my name on the line below and submitting electronically, it represents my signature and acceptance of the above.
